Physicals and Other Important Information

A Virginia High School League physical and Orange County Middle School Activities Waiver forms are required prior to participating in conditioning, tryouts and participation in all athletic teams.

  • Please note that physicals are valid for 14 months from the date of physicians' signature on page 3. 
  • Waiver forms must be completed yearly. 
  • The physical must be completed before participating in any middle school or high school sport that is sanctioned by VHSL.
